A report that is easy to assess
Use a subject such as Swim speed: training prompt changed. Include the exact sentence or table entry, a readable source, and the observation date. For an earning or upgrade claim, describe the conditions and the result you actually saw. Keep one disputed fact per report so its evidence and effect are clear.
